Trinity Healthcare Talent Services is seeking a Travel Occupational Therapist (OT) for a home health role in Rockford, IL .
This position focuses on delivering therapy services directly to patients in their homes, supporting independence and functional outcomes.
Active Illinois OT License
BLS Certification (AHA)
Minimum 1 year of OT experience required
Home health experience required
Must be comfortable with independent field work
Schedule: Monday–Friday | 8:00 AM – 5:00 PM
Rotating weekends required
Duration: 13 weeks
Setting: Home Health / Home-Based Primary Care
Mileage: Reimbursed between patient visits
Home-based care model
Focus on improving patient independence in daily living
Work directly within patient homes
Independent caseload management
No local candidates (50-mile radius rule)
Must have reliable transportation
Mileage reimbursement provided
Requires strong time management and documentation skills
