Position Requirements

Bachelor of Science from an accredited school of pharmacy with current licensure as a Registered Pharmacist in the State of Florida. Minimum of 1-year experience in the field of intravenous admixture, of all types. Be able to plan and communicate, both verbally and in writing, with other health care professionals and assume responsibilities in the monitoring and management of nutritional support and drug therapy.

Position Requirements

Bachelor of Science from an accredited School of Pharmacy with current licensure as a Registered Pharmacist in the State of Florida .

Background and knowledge of pharmaceutics and their drugs and interactions.

Minimum of one year experience in the field of intravenous admixture, of all types.

Excellent verbal and written communication skills, including strong organizational, detail, managerial and interpersonal skills.

Experience with pharmacy management systems required (PrimeRX preferred); familiarity and working ability with practice management systems, database operations, spreadsheets, and other software systems is a plus.

Assumes responsibilities in the monitoring and management of nutritional support and drug therapy.

Duties and Responsibilities:

1. Oversees the initial patient evaluation as it pertains to the therapy prescribed.

2. Oversees the collection, analyzing, interpretation and evaluation of appropriate data relative to the patients therapy. Oversees receiving and documentation of medication orders.

3. Oversees the obtaining of histories and application of these along with other patient data to optimize nutritional support and/or drug therapy management.

4. Oversees sets of therapeutic objectives and monitoring of patients for desired therapeutic responses.

5. Responsible for 24-hour availability as required ensuring operational compliance.

6.Oversees the evaluation of the significance of signs and symptoms relative to certain disease processes and makes recommendations for the modifications of nutritional support and drug therapy.

7.Oversees the detection and evaluation of the clinical importance of adverse reactions and complications associated with home IV therapies.

8.Oversees when the pharmacist provides counseling and answers questions from patients, family and health care professionals concerning nutritional support and drug therapy.

9.Oversees the maintenance of thorough and appropriate documentation in the patient clinical chart of physician orders and communication with other health care professionals.

10.Oversees the pharmacist when assisting the referring physician and institution in planning and implementing patient care to ensure a smooth transition from hospital to home.

11.Oversees the participation of Team Conferences, Care Plans, Medication reconciliation, lab values within pharmacy policy.

12.Maintains proficiency in the skills and theory of nutritional support and IV therapy by reviewing the literature and attending in-services, seminars and conferences.

13.Supervises and coordinates activities concerned with infusion services: ordering, receiving, storing, inventorying, issuing, and shipping materials, supplies, tools, equipment, and parts, in the stockroom and warehouse.

14.Oversees the preparation of mixing of sterile IV products such as TPN, chemotherapy, and antibiotics.

15.Oversees the pharmacist and pharmacy technician(s).Participates in the training of staff and monitoring their performance and progress (relation to infusion therapy).

16.Oversees the pharmacists verification of accuracy of all work done bypharmacy technician(s).

17.Participate in the Companys Performance Improvement program and participate in Company committeesas requested by Administration/Director of Clinical Services.

18.Ensurecompliance with the organization's Code of Conduct and all applicable laws, including HIPAA regulations for the protection of sensitive information.

19 Perform any additional duties as assigned to meet the needs of the department and organization.
